If a pediatric ortho specialist sees a healthy child minus injury/ortho condition. How should the physician dictate PFSH without it seeing as if it is cloning? He ususally ask the same questions for each patient.
The best way to do this would be to ensure that any positives are mentioned - after all even healthy kids may have something that is being followed, monitored, whatever. Or, if all are negative, mention one or two, such as no anxiety or depression for psych, whatever. Don't have to go too crazy with this, but since it sounds like your concern is the appearance of cloning, just mix that up every once in awhile.
