I am new to the field of DME billings, hope could get some input from you.
I am working for a podiatrist who recently join the program for DME services which she can submit claims for special dressing gauze sponge and bandage. As per the DME vendor advised, when submitting claim, the place of service should use 12 as "home", because product was delivered to patient's home and be used at patient's home. However, physician rendered service in the hospital as inpatient visit, should I still use 12 as the place of service? Then what information I should use for Service Facility Location Information (Box 32 a & b) on the claim form? Should I use the physician's office address? Or leave it blank?
