Wiki DM without and with complications

If a person has Assessment of 1.DM and they also have 2.Polyneuropathy, and provider also mentioned in PMH 1.DM w/o Complications,2.Polyneuropathy,3.CKD,
Is there still the relationship in the new guidelines between the two(DM with subterm DX). Report does not mention CKD in assessment.

Please kindly clarify I can code E11.9 , G62.9 & N18.9 separately ,otherwise code it as combo E11.42??????

I agree with you that you should use the E11.42. You need to find out if the CKD is indeed related to the DM because if it is then you'll need E13.22 along with the CKD stage code. Good luck

You need to read the guidelines and follow them. In the Alpha section the main term diabetes is followed by the term "with". Any condition listed under the term "with" is an automatic causal relationship and does not need to be stated by the provider. So it would be coded as e11.42 and E11.22 with N18.9.
