The record documents "DM with renal manifestation" in the assessment. No specificity as to what the renal manifestation is, would you assign 250.40 alone, or would you assign another code for renal maifestation. ALL ANSWERS ARE WELCOME!! Thank you

you must also assign the manifestation code. The physician should be quieried to add this to the documentation.

No, you can't code the DM with renal manifestation code alone. In my opinion, this needs to be taken back to the physician for clarification. If the doctor did not mention anything else about the renal manifestation, chances are, the doctor did not manage/treat the condition during this encounter and therefore only 250.0X should be coded. If the doctor did specify some med changes or review and management of the manifestation, the doctor needs to clarify to get credit for this.
