Wiki DM Diagnosis?

Hi All,
Provider coded this account with BOTH Diagnosis: 1.) R73.09 Other Abnormal Glucose and 2.) E11.9 DM with lab result of;

Diabetes;
HGBA1C increased from 8.7 to 10.1


Is it correct please advise!

Coding has to be based on documentation, so without seeing the documentation, there's no way to say if this is correct or not. But normally, 'symptom' codes, such as R73.09, 'that are associated routinely with a disease process should not be assigned as additional codes', so you would not usually see this code reported together with diabetes. But again, it would all really depend on what is in the note.

R73 category has an excludes 1 note for all E08-E13 codes. So there is the definitive answer. You could bypass the edit with an overriding reason but there really is nothing here you can use to over ride this edit, the patient only has one endocrine system.
