Wiki DJD lumbar with Spondylosis

I just wanted to check myself here because I am light on documentation. I am looking for both ICD9 and CPT codes, if possible.

.....Patient comes in for outpatient services. Physician order states low back pain and lumbar radiculopathy. X-ray of the lumbar spine shows degenerative disk disease of the lumbar spine with spondylosis.

What are my ICD9s?....this is not in Radiology dept, so I am not even sure if I can code the Xray cpt. Please help.

Need a bit more information. Is that the entirety of the note? Is this is an outpatient follow-up or initail visit?
724.2 Low back pain
724.4 Lumbar rad.
if he documents he read the x-ray, you can use diagnoses
722.52 DDD lumbar spine
721.3 Spondy lumbar region.

We are in a Hosp system, Radiology bills the X-rays, we bill the E/m only.
