Wiki Discharged from Psych to ICU ??

Patient is in Psych floor. Not eating well and becomes malnurished and has to go to ICU for medical purposes. Is the last note by the Psych doc considered a discharge? Or is there another code that is used for this situation? Thanks for your input.
