We have a Medicare patient who just had a knee arthroscopy done. Now she is needing carpal tunnel surgery and she is still in the global period. Will we be reimbursed at full Medicare allowable since it is a different part of the body. The doctor seems to think he will not be paid the full fee. Any help will be appreciated.

For the surgery add modifier 79. If the patient is coming in for an E/M for her CTS, use modifier 24 for those office visits. You should get full payment for the surgery. If denied initially as global to the first procedure (which they shouldn't if the modifier 79 goes with it), you'll need to appeal the denial with a letter and send a copy of the operative report for the carpal tunnel
