With the 66982 we need to use 2 diagnosis for the reason it is complex,, What DXs would you use for this one?
Example
Please code as a "complex" cataract surgery for the following reason: Due to the patient's dense, whitish -rown, mature nuclear sclerotic cataract, it was necessary to use trypan blue dye to enhance the anterior capsule for cataract surgery.

Denials on 66982

Has anyone been getting denials for the complex cataract 2nd. diagnosis?
 
Only H25.1 series code is enough to code for complex cataract in this scenario. There is no such need for the 2nd Dx. 66982 got denial only in case of H26.9( unspecified cataract). If there is any specified cataract then there is no need for 2nd Dx. Try to search in op note is there any kind of miosis. If its there then we can use it as 2nd Dx.:eek:
 
When my physician does a 66982 it is usually because of something like floppy iris syndrome. I use the cataract code first then follow it with the floppy iris code. The same would be true if it was for any other situation where there is another issue that makes the surgery complicated.
 
Complex catarct denial

But according to the LCD for Cataract Extraction, you must use a code from Group 1 and group 2.
 
Secondary diagnosis

According to the LCD, you would report H25.89 Other age-related cataract as secondary with the use of trypan blue dye was used to stain the anterior capsule (see the General Information section below the list of diagnosis.)
