Wiki Diagnosis clarification...help

I code and bill for a testing facility so the diagnosis' on the orders from the doctor's offices are sometimes vague...

I am wondering if I am coding correctly a DX:

CAD s/p stents....I have been coding 414.01 since there is no bypass mentioned, but wondered if I should be doing something else. Is V45.82 appropriate if the s/p PTCA is not on the order?

Any suggestions and/or explanations to help would be appreciated.

If it is not specified it should be 414.00, however for ICD-10 CM it will need to be specified in the documentation. YOu only use the V45.82 when it is documented in the progress note for the encounter.
