Wiki Diabetes with or without complication

I have a patient who is not on any meds for diabetes and only gets their A1C checked monthly in an MDs office. The patient also has a HX of renal failure and goes to dialysis 2x a week. Should I be using 250.00 or 250.40

the provider must state that the renal failure is due to the diabetes or diabetic renal failure before you would assign the 250.40 code
