Coders please help, do you have to report the manifestion code right after the diabetes code.

Could someone please tell me the correct way to code this chart? The ICD-9 guidelines states the manifestion code has to be sequences following diabetes code.

Dx code 250.42 = renal manifestion, does the patient have a renal condition? If so, you would code first 250.42 then the renal manifestion (example: 250.40, 585.9). If the patient has Neuropathy, code first 250.6? then the neuropathy manifestion code (example: 250.60, 357.2).

I am a bit confused by your code choices , You have diabetes with renal complications, but no renal code and you have a polyneuropathy in diabetes code with no diabetes with neurological complication code.
Of course the code you sequence first on the claim would be the one that matches why the patient is there. if that reason is the polyneuropathy as a manifestation of the diabetes the it is correct to list the 250.62 first listed followed by the 357.2. by doing it this way you are saying the diabetic manifestation is specifically the polyneuropathy.
