Big debate gong on in our office over coding dexa scans. Doctor orders the scan as a screening, but the scan reveals osteopenia or osteoporosis. The scan is then billed with the problem dx, so the insrance company does not see as screening and will not pay the same benefits. Patient gets mad and says doc says it was a screeniing? Should you not code to the finding when a problem is found?

The screening ICD-9-CM code should be listed first.
Conditions discovered during the screening may be listed as additional codes.

This is based on official ICD-9-CM guidelines. Hope it helps.

(P.S. - Be sure to check carrier policy when available, just in case they require a different method of reporting.)
