The doctor excised 5 benign skin lesions of less than 0.5 cm from the trunk and closed each with an intermediate/layered closure. How should this be coded?

each lesion is listed as a separate line item with a 59 the repair is the sum of the lengths which is 2.5cm which codes out 12031. I hope this will help
 
Surgical wound

It's entirely possible (in fact, likely) that the length of the surgical incision is larger than the size of the lesion (just look at the diagram on pg 54 of 2009 CPT Professional Edition.)

The physician should be dictating the length of the repair separately from the size of the lesion ... s/he is leaving money on the table if not doing this.
