This is what your mod 22 is for. Have your doc dictate cover letter explaining why it was more difficult, how much extra time involved.etc.......send cover letter with op report, they will pay higher than normal on your code choice.

If "different doctor" is in the same practice then it may be a little different. Other than that modif 22 should work.
