In the assessment it says Depression 311 but in the HPI it says anxiety with depression. What would you code?

I would just code the depression...unless the anxiety was brought up in the assessment plan, which it sounds like it wasn't.

311 is not Code for depression. It is a code for depressive disorder. You need to be certain that the provider has intended to diagnose the patient with a depressive disorder befor appending this code. There are coded in the symptom chapter for demoralization and apathy and in ICD-10 CM a code for unhappiness. Depression is not the same thing as a depressive disorder.
