Wiki Depo-Provera Rejections

Hello, has anyone else having issues with their 2024 Depo-Provera codes? Just last year they have been rejecting. We usually bill the insurance J3490-U8 (our office purchases the medication - "Buy and Bill") with;
NDC listed: N466993337179
Unit Qualifier: ML0000001000.
Depo-Provera 150 mg

(IEHP- Medicaid)

Is there a reason you are billing unlisted J code instead of J1050? That would have to be my first guess.

According to the insurance (IEHP Medicaid) this is the code required for "Buy and Bill"

I never heard of a plan wanting you to use a less specific code, but even after 25+ years, I hear of new things. I also notice for your NDC#, you have it starting with "N" and then 12 digits. Again, unless that is a specific instruction for this payor, NDC format is typically 11 digits. https://www.drugs.com/ndc.html

I found out our NDC was incorrect I did update it but i was wondering if you would know where we would add the "0 filler"
NDC: 66993-371-79

for example:
1234-1234-12
(01234-1234-12)

12345-123-12
(12345-0123-12)
