Some coworkers of mine came across an issue that this patients insurance company is "bundling" CPT code 99360 to 99465. I looked in the CPT manual and it states that those CPT codes can be billed together but the insurance companies "claim check" program states to disallow 99360. I'm confused as to why this would happen. I played around with the "claim check" program and added on modifier 25, 59 etc. and it still said to disallow. I'm confused by this because I am not yet certified and still learning, the manual says you can report both so what am I missing? Would greatly help me for future reference.
