Patient presents to thephysicians office with a deformed toe that is causing him pain. After examination the physician documents that the patient has a toe deformity with a ulcer with swelling and erythema from rubbing due to the deformity. The physician states in his plan that the patient has a acquired toe deformity, ulcer of the toe and diabetes. Awaiting referral for Surgery of the toe deformity. Due to the diabetes the patient will be followed closely for infection and non healing of the ulcer.

I would code as ulcer due to deformed toe complicated by diabetes. The patient is not seeing your doctor for diabetes, but for pain/ulcer.
