Wiki CVA due to intracranial head injury per past medical records

Hello! I am in search of some advice in regards to adding S06.9X0S (Unspecified intracranial injury without loss of consciousness) to a patient's chart in addition to the CVA and late effect codes. The CVA was caused secondary to a head injury per past medical records, however, the records do not state that the patient lost consciousness or has anything specific as to how it occurred for the additional codes. Would you add this code to I67.89 and/or I69.954 for late effect hemiparesis in this case?
