When is it appropriate to bill for the orthotic? When the casting is taken or when the pt receives the orthotic?

Actually, you may have it a little backward. CMS rules state that you have to bill it when the patient has the supply IN HAND, so you should be billing a no charge visit for the fitting, and the actual cost on the date of delivery. Make sure it matches your signed "Receipt for Delivery" which is also a requirement of CMS. I figure if Medicare wants it that way, so will the other carriers, and besides, it makes us consistent.
 
The last office I worked at we did custom orthotics & prosthetics. We billed out for the date that the patient received the DME and kept a "delivery slip" that was signed by the patient in their chart as well.
