Chewing tobacco is not indexed in ICD-10 the same as cigarettes. While there is a code for chewing tobacco DEPENDENCE, the correct code to report is Z72.0 for chewing tobacco use. The chapter specific guideline for use/abuse/dependence still applies to nicotine, whethere it is inhaled when smoked, or swallowed when chewed. The distinction is - SMOKER indexes directly to tobacco dependence, and chewing tobacco does not.

When the documentation states the patient chews tobacco and nothing more, the only code that is supported is Z72.0 for tobacco use. The provider must state chewing tobacco "dependence" or some form of that word (dependent/addicted/etc.) to report the F17.220 code. People that chew tobacco are not classifed by ICD-10 as tobacco dependent.
