Hello, Could use an opinion as to why the payor has denied the code 68100? Coded 68725-50 and 68100-LT, know the cryotherapy is per procedure but they paid for the bilateral procedure but denied the conjunctival biopsy which I don't understand. Can someone explain? Procedure: cryotherapy was done on the right lower lid, left lower and left upper lid. Conjunctiva biopsy was done on the left lower lid.
