Wiki critical care time

I know for critical care that if it is less than 30 minutes it is a subsequent visit. What if doc did 2 hours of critical care but didn't include total time in note? Does it drop to lowest level of critical care or subsequent hospital level?

If time is not documented you cannot bill even the lowest level of critical care since at least 30 minutes must be documented.
