Wiki CPT Code Help, Please?

I am wondering how I would code a lateral capsular disruption? The physician states he did a Synovial Cyst aspiration, L5-S1 intra-articular facet block, and lateral capsular disruption. I have the 20612, 77002, and 64493, but not sure if the disruption is appropriate to code? Thanks for any advice offered!

Is that something he did or the reason the other 2 procedures were done? Do you have a report with the description of this procedure?
