Wiki CPT code 35301 vs 35511

This is general question and maybe I am missing something with regards to the use of these two codes.

Can someone explain the difference in the two?

CPT code book shows:

35301 - Thromboendarterectomy, includes patch graft, if performed; carotid, vertebral, subclavian, my neck incision

35511 - Bypass graft, with vein; subclavian - subclavian
 
35301 - This code is where the thrombosed segment of artery is isolated, artery is cut open and thrombus removed. The vessel is then sutured closed. Cannot bill separately for repair of vessel.

35511 - This code is when a portion of a vessel has a stricture (either stenosis or thrombus) and that portion of vessel is bypassed using a vein segment harvested from another location. You can report the harvesting of the vein segment separately as long as it is not harvested from the saphenous vein.
