Wiki CPT Code 15271 reimbursement

We are billing cpt code 15271 with pos 32 but have not received payment from Medicare. They are rejecting it with CO-16 Claim/service lacks information or has submission/billing error(s) which is needed for
adjudication. Anyone else having this problem?

Please help!
