I am looking at denials from Medicare stating: *PYMT ADJUSTED B/C PAYER DEEMS INFO SUBMITTED DOES NOT SUPPORT THIS MANY SERVICES. We are billing 36475-RT and 36475-LT. Is this correct for the 1st vein per site? Or should one be billed as a subsequent vein CPT?
