CPT 2012 CONVERTED CPT CODE 29826 TO AN ADD-ON CODE TO BE LISTED SEPARATELY IN ADDITION TO THE CODE FOR THE PRIMARY PROCEDURE. IT SPECIFICALLY INDICATES THAT 29826 IS TO BE USED IN CONJUNCTION WITH 29826-29825, 29827 AND 29828.
CCI EDITS INDICATE THAT 29826 CAN BE REPORTED WITH CPT 23412 BY APPENDING MODIFIER -59.
WHICH GUIDELINES ARE APPROPRIATE TO FOLLOW???

I took the notation in the CPT book to be that 29826 is an add-on to the other shoulder arthroscopic codes listed. I also had a case that had the 23412 for rotator cuff repair but on the arthroscopic side of the surgery they also debrided the labrum. I used the 23412 and 29822 and did not use the 29826, as the CCI edits edited out the 29826. There was no CCI edit for the 29822 coded along with the 23412, so they both worked together. I debated over this case for quite a while also. This new change can be confusing.

Thanks for your input, I think the CCI Edits are incorrect with the indicator 1 showing 29826 as a component of the 23412 when it is now an add-on code with the specific instructions to report it with the other arthroscopic codes. Unfortunately, my physician only performed the 29826 arthroscopically so no other code to bill except the 23412.
