When billing 19125, 19285 & 19296 most carriers denied 19285 & 19286. Should I append 51 or 59 modifier to 19285?
 
I add modifier XP, Separate Practitioner to both 19285 and 19286. A Radiologist performs the Needle Localization and the surgeon performs the excision/surgery.
