Wiki CPT 11102 & 11103 denying as in global/apart of another procedure

Hello,

I have a few claims from particularly Premera and Amerigroup - but they are denying CPT codes 11102 & 11103 when billed together, even with no office visit or other procedure. I called in to ask and I am told there is a CCI edit but I find no edits between these two codes. Is anyone else having this issue?

11102 & 11103

We are getting denials for 11103 when billed out with 11102. I'm presuming we are doing it like the 11100 and 11101. Can someone tell me what I am doing wrong?

OK! We heard from Medicare about 11103, it has to be on one line and change the units. Even though Encoder states each be listed, Medicare and commercial insurances are accepting with one line. I now have about 12 denials to be done on the IVR... not fun.... but now I know!

New BX codes

We are having the same issues. Billing example-- 99202-25. this is how its billed out on claim. Medicare just came back with a denial. Can you see what I'm doing wrong. They pd OV and primary BX code and denied add on code
