Hello! We are going to start in house testing/billing for COVID 19 as a pre-admission criteria for our residential services. We will be billing 87635 QW (we have CLIA waiver) for the test. I am STRUGGLING to determine if a rendering provider be required in Box 24J of the HCFA? If so would the rendering be the physician who reviewed the results and signed off, the ordering, or the overseeing practitioner of the residential program? I have searched through the AMA guidelines and I couldn't find billing guidance for the rendering for a pre-admission test that doesn't include an evaluation and management service. Thank you in advance for any information you may have.
