You shouldn't need a modifier. This is an each code so if 4 separate tendons then it is 26145x4. The tenosynovectomies were done at the level of the palm not in the finger. I would however advise that there be documentation to support that these were indeed radical tenosynovectomies and not simply creative documentation for trigger finger releases which also involve a bit of tendon clean up.
