In our office we have a multi-dose vial of Depo-Medrol 80 mg
MD orders 120 mg IM injection
There is conflict in our office how to report the drug to Medicare (PA MAC).
Do we report J1040 (80 mg depo) x 2 units OR J1040 (80 mg) x 1 unit & J1030 (40 mg) x 1 unit? For some reason, staff in our office think that the NDC must match the J code when reported on the claim.
Does anyone have CMS guidelines related to billing for multi-dose vials? I wasn't able to find anything specific to help with my question.
