The planned procedure was for an IM rod 27244 for a subtrochanteric femur fx, but during the surgery doc had to switch from a standard IM rod to a trochanteric cephalomedullary fixation rod, and then due to soft bone, had to proceed with an open reduction internal fixation with a compression plate.

Do I code 27245 with mod -53 for having discontnued that procedure and then bill out the 27244,

or do I just bill the 27244 with mod 22 for the increased work? Thanks so much in advance for any responces. Mary MMadrigal@hawaii.rr.com

If he used the cephalomedullary rod you would use 27245 because cephalomedullary rods are antegrade intramedullary rods used to repair femoral fx. I code for ortho trauma and my docs use them all the time.
