Can a provider bill for a consult when first seeing the patient in hospital, then bill another consult when the patient is seen as a new patient in a rehab facility by the same doctor?
 
First, if this is Medicare he cannot bill a consult at all. In the hospital lhe would bill 99221-99223 or 99231-99233 depending on the documentation. If the patient were an out patient or in the ER the appropriate outpatient/ER codes would be billed.

Second, if the doctor saw the patient in the hospital, he/she would not be a new patient at the rehab facility, he/she would be established.

That being said, if the patient was NOT a Medicare patient and your doc received a request from another doc to evaluate the patient at the facility, then yes he can bill another consult. The big IF here is; was there a request to see the patient, or was it just a follow-up from the hospital.
