Hello and thanks to anyone who replys :)

specimen referred from outside institution...88321

are gross and microscopic exam required?

report has appropriate pt demographics, tracking identifications, referring phy name and institution, a final diagnosis (which I am understanding infers microscopic exam) but no gross description.

Is it necessary?

Also, can any other CPT codes be submitted for that exam, in addition to the consult code.
Or does the consult included gross and microscopic examination?
 
Our pathologists will include a gross that describes exactly what was received for the consultation: slide, block, and from whom.

Typically on our consults, the pathologist will include a full microscopic report. There needs to be some sort of documentation that the microscopic was done, whether a full report or just a report that it was done.

88321 is inclusive of what you listed, but additional codes can be used, for example, if special stains were done. In that case, though, the stains will need a -59 modifier.
