I am looking for clarification.
A consultation requires the written or verbal request for consult may be made by a physician or other appropriate source and documented in the patient's medical record by either the consulting or requesting physician or appropriate source. The consultant's opinion and any services that were ordered or performed must also be documented in the patient's medical record and communicated by written report to the requesting physician or other appropriate source.
Is an electronic referral acceptable documentation to support the request? Is a comment in the reason for visit such as ,Dr. Smith referred patient for evaluation of chronic sore throats, acceptable?
If a copy of the progress note is faxed to the referring physician, will this support the requirement for written report communicated back to the referring provider?
