We are a pain management group and payers require we show them proof of three months of conservative treatment, i.e. Physical therapy, home exercise, NSAIDS, etc.. But does anyone know the time limits on conservative treatment? For example, if the patient came in 2 years ago and conservative treatment was established and failed, but patient opted to wait on injections then, but wants them now, does the conservative from 2 years ago qualify or does it need to be repeated? I cannot find documentation to support how recent conservative must be done.
