Is it ok to bill insurance if the patient has not signed release of information, consent for treatment and privacy policy forms?
 
Patient must sign the authorization for you to bill insurance, unless your insurance contract states otherwise. You can't treat the patient if the treatment requires a consent, such as surgery, but if it's an office visit, you are operating under 'implied consent", i.e., by coming to the office the patient gives consent. You must provide HIPAA privacy policy info to the patient and make an effort to obtain a signature, but the signature is not required. Hope that helps.
