Wiki Confusion on transforaminal coding

Hello everyone, I have a question on how to code injections done in our facility yesterday. The doctor dictates he preformed a transforaminal at L2-L3 on the left and then the a transforaminal at L4-L5 on the right.

Would this be coded:
64483 LT
64484 RT

or

64483 LT
64483 RT

Any help would be greatly apperciated!
 
64484 represents an additional level performed. Whether it is right or left for the additional level should not exempt the reporting of the add on code.

The correct reporting is

64483
64484 with additional note stating separate level contralateral side
 
Since the procedure is performed at 2 separate levels the add on code is appropriate to capture how it should be reimbursed. The reimbursement should not differ whether you perform a L4-L5 RT L5-S1 RT or whether the L5-S1 is performed on LT side, it still represents a two level procedure.

64483 RT 64483 LT would be processed as a bilateral procedure at 150 percent which the service was not a bilateral procedure at a single level.
