I have a patient who received 2 non-chemo infusions, both started at 12:00, one ending at 13:00 & the other ending at 14:00. Should I use 96365 and 96368 or 96365, 96366 and 96368?

Question for you...why do you need 3 codes when only two non-chemo meds were given? Why wouldn't just the initial and the concurrent be charged (96365 & 96368)? Just curious and you are my best source for learning injections & infusions!
