Patient diagnosed with prothrombin gene mutation (289.81) and is 22 weeks pregnant. Dr gave diagnosis of prothrombin gene mutation and will initiate heparin at week 36.

My question is do I code as 649.30 (coagulation defects complicating pregnancy) with 289.81 or 289.81 and V22.2? MD made no mention of it being a complication but is it safe to assume it is?
 
According to your coding guidelines, it is always a complication of pregnancy unless the doctor indicates otherwise.
 
Use 649.33 followed by 289.81 since the patient is 22 weeks pregnant (Antepartum). Any condition associated with pregnancy must be coded with Chapter 6 codes as Pdx along with additional codes of any chapter as applicable.
