I wanted to get compliance feedback for a policy that is currently in place at my new job. This policy seem to me as a concern but since I'm new to Managed Care environment would like my fellow compliance community feedback.
The organization policy currently in place are for addendums. For patients that were visited within the past 30 days the coding team will create an addendum note with the diagnosis along with the assessment and plan from problem list and send it to the PCP. The PCP will review diagnoses and plan for accuracy, edit or remove diagnosis or plan and sign off.
These patients are identified from an automated analysis,
My concern is the function of the coding team with documenting the diagnosis and plan were a PCP could just sign off. My concern would be also the accuracy of documentation after 6 months.
