getting familiar in coding for gastro, can anyone be my second eyes here, am I capturing appropriate codes.

CPT: 45381, 45385 or should it be 45381,45388

There was a 13mm broad based polyp located in the descending colon s/p injection with 8 cc of a pre-mixed solution of 2 gtts methylene blue, 10% 1:10,000 epinephrine, 90% sterile saline and polypectomy using electrocautery snare with the ERBE unit on the custom EndoCut Q settings with an extended interval of 6
Removed in one piece. No clips used.

45385 ( no 45381 ..., the injection /premixed solution is bundled in to lift the polyp from it's base)

I'm sorry, NCCI doesn't show as bundled, is there another guideline where it states its bundled?

Yes, they can be billed together, but based on your op report, they used the saline to lift the polyp to be excised and not just marked it for future area to watch.
