If a physician removes a colon polyp, let's say by snare, but is unable to retrieve the polyp, how do you bill?

There is no path report, so no way to know if the polyp was adenomatous or hyperplastic. So which would you choose? Some physicians believe you would bill it as adenomatous and recall the patient in 3 years, while some sources state you bill as hyperplastic, which would have a recall of 10 years. Does anyone have any sources to clarify this?
