reading CMS guidelines for colonscopy coding, it says to use V67.09 as a prim dx and the personal history of colon cancer V10.05, polyps V12.72 and V10.06 Personal history of malignant neoplasm of rectum rectosigmoid junction and anus as secondary codes. With this, where do other dx codes come in such as new polyps 211.3, diverticulosis 562.10. Are we using what is found 1st or the V codes 1st. Is any of this right at all? Help!

In this case the V67.xx code would be listed primary and then I would put the findings followed by the actualy history.

So:

V67.09, 211.3, 562.10, V12.72
