I had a commercial insurer deny a colonoscopy (screening with a family history of colon cancer) because I didn't use the Medicare G code for high risk patient. This is a first for me. Are the G codes also for commercial insurers or just Medicare?

Some of the commercial payers require G codes for screenings as well. All Medicare and Medicare replacements use them.
