Please help me understand the question on the Coding Edge Test Yourself?
Question reads in essence: pt has abd pain and diarrhea. Dr performs screening colonoscopy. found polyps, which were removed.
Answer: DX = polyps, and signs and symptoms are not reported ....and report proc as polyp removal ...

But, in the article in the Coding Edge, on page 27, under Rationale, it states: when coding, diagnosis symptoms are sequenced first, and findings last.

Do you report the abd pain and diarrhea, or not?? Only the polyps??

Part B questions answered says to code the reason for the colono, then the findings. So yes you would report the abd pain and diarrhea first, then the findings secondary.
