Can someone please tell me the proper way to bill this scenario to Medicare?

- EGD with biopsy
- Colonoscopy with hemorrhoid band ligation (physician states surveillance. Pt. had colonoscopy in 2011 but is having some rectal bleeding and has hx of polyps)
